The Empire Plan seems to be denying all 2015 claims for patients who are the spouse or dependent of the insured. They are doing so stating that they are requesting confirmation that the patient isn’t eligible for coverage under another insurance carrier. In such cases, the insured would need to contact Value Options to update the Coordination of Benefit information for all family members covered on the plan. Typically, an insurance will take this action when they have legitimate cause to believe that a patient has dual insurance coverage. Given that this appears to be an across-the-board fishing expedition, I would also advise the policy holders to bring this to the attention of their HR reps.
